If there are any changes to your child's medical needs, please complete the Medical Information form below and return it at your earliest convenience for us to update your child's details.
If your child requires medicine in school hours, please complete the Timberley Academy Medical Form below. Please bring it in to the school office for us to be able to update your child's details and inform the relevant staff members to administer accordingly.
Please do not give your child any medication to keep with them during the school day.
