What happens when your child moves year group, phase or to a new school.

At the end of each school year class teachers hold transition meeting to discuss the needs of the children in preparation for their move to a new year group or phase.

 

If your child moves to a new school then the SENDCo from both schools make sure that any necessary information is shared.

 

If your child is starting with us in reception and did not attend our nursery then we will talk to their previous nursery to find out as much information as possible about their individual needs.

 

We arrange a home visit for all children who start our nursery and complete a personal profile; this is information from you about your child’s interests, likes and dislikes.

 

When your child moves from year 6 to secondary school the SEND team will send all relevant information regarding your child’s special educational needs. Extra transition visits can also be arranged.

 

For children who have an EHCP then we will hold a transition review with all agencies and their named secondary school to ensure a smooth transition.
